Village West Vikings

As Parent/Guardian of the above-name minor(s), I grant permission for the swimmer(s) to participate in all activities of the Village West Vikings Swim Team, a Northern Virginia Swimming League (NVSL) member team. I represent and warrant that my minor child/children participating on the Village West Vikings Swim Team are in good health and have no physical condition, ailment or disability which renders them unable to participate in vigorous physical activity. For and in consideration of benefits derived from participation in the Village West Vikings Swim Team program, I understand that the risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. I assume all risks and hazards arising out of, or related to, such participation, including, but not limited to, transportation to and from such activities, and do hereby indemnify, release and hold harmless the Village West Vikings Swim Team, its coaches, team representatives, volunteers, property manager, employees and agents, as well as the Northern Virginia Swimming League, from all claims of any kind whatsoever which may arise or hereafter accrue in connection with my child’s/children’s participation in activities of the Village West Vikings Swim Team. I further grant permission for first aid to be given to my child/children in an emergency and will be solely responsible for any medical costs which may arise. I AGREE THAT THEY WILL ABIDE BY THE NORTHERN VIRGINIA SWIMMING LEAGUE CODE OF CONDUCT.

I certify that I am a participant age 18 or the parent or legal guardian for my child(ren).

Release of Liability to Participate

I understand that participation in swimming activities involves a certain degree of risk. I have carefully considered the risk involved and have given consent for myself or my child to participate in these activities. I understand that participation in these activities is entirely voluntary and requires participants to abide by applicable rules and standards of conduct. I release the Village West Swim Team, its representatives and coaching staff, Village West Inc. and its Board of Directors, Fairfax County Park Authority and its staff, the activity coordinators, and all employees, volunteers, related parties, or other organizations associated with the activity from any and all claims or liability arising out of this participation. I approve the sharing of the information in this registration with the coaching staff and professionals who need to know of medical situations that might require special consideration for the safe conduct of swimming activities.

By accepting this agreement and registering myself or my child(ren) for the Village West Vikings, I acknowledge that I understand and accept the above terms.

In order to keep our season as organized & successful as possible, we, the Vikings coaches, ask you hold the following standards. It’s crucial to the success of your child!

• Please have your swimmer arrive 15 minutes before practice begins and be ready to enter the water at the scheduled start time. This means having their swimsuit, cap, and goggles on and ready to go.
• Please stay off the deck near the lanes during practice. Parents on the deck can create distractions for swimmers, especially younger ones. In order for your child to get the most out of practice, we need to minimize distractions.
• If your child is upset, crying, or having any difficulty during practice, please allow our coaching staff to assist them. As coaches, our goal is to build a supportive and routine relationship with your child. Feel free to communicate with us about any big changes, emotional growth, or family events so we can be sensitive to the needs of your child over the course of the summer.
• Feedback about strokes and constructive criticism about races should come directly from the coaches. We will always be encouraging while providing developmental guidance to support your child's growth!
• Please respect the coaches. We know your swimmer’s abilities and potential. We will never put them in an event or teach a new skill unless we are confident in their ability.
• You are your child's biggest support system. Please encourage them and celebrate their “personal best” times! We aim to create a fun, loving, and exciting environment for all swimmers. Mistakes, disqualifications, and losses are a natural part of growth. Help us encourage your child to keep their chin up and try again. Remember, no one is perfect – every experience is an opportunity to learn.
• Show up, cheer them on, and be there to support your child. Love them for who they are, not just for their accomplishments.

I hereby grant permission to the Village West Swim Team to take and use photographs and/or digital images of our family, including registered swimmers, adult volunteers, and meet spectators, for use in news releases and/or promotional materials. These materials might include printed or electronic publications, web sites, or other electronic communications. I authorize the use of these images without compensation to us. All negatives, prints, digital reproductions shall be the property of the Village West Swim Team..
